You MUST include:

1. Application Form & Package (see below)
2. Your passport
   
3. Two passport-style photos per application
   
4. Proof of travel (flight itinerary, copy of plane ticket, etc.) showing entry into and exit from intended destination
5. Additional documentation as required (see below

***Non-U.S. citizens must also send a copy of their proof of status in the U.S. (i.e. front and back of green card, visa, application for status extension, etc.)
